Medical Certificates

In order to obtain your FAA Medical Certificate you will need:

MedXpress form and confirmation number

• Log on to medxpress.faa.gov/medxpress to get the MedXpress application. Once submitted you will get a confirmation number and a pdf copy of  the application that you submitted.
• Email the confirmation number and the pdf copy of the submitted MedXpress form to berckes.aeromedical@gmail.com 

If you have a medical condition or take any medication, there could be implications to obtaining a medical license. Please review the list of medications here. This includes high blood pressure.

After the documents are received, we will email you with the date and time for your appointment.

To start the HIMS process 

Submit the following :

• Legible copy of your driver's license or passport
• Your most recent Medical Certificate
• Any letters from the FAA, medical reports, and police reports as applicable
• A personal narrative about what has occurred requiring HIMS services

Send all documents in .pdf format, in a single email to berckes.aeromedical@gmail.com

We recommend the documents be password protected or encrypted.

DO NOT COME IN TO THE OFFICE IF YOU ARE SICK, COUGHING OR HAVE FEVER. Send an email to berckes.aeromedical@gmail.com or call 321.389.9916 to reschedule the appointment.

We accept payment via Venmo, cash, and check (with valid Florida drivers license).

CREDIT CARDS ARE NOT ACCEPTED
